Open Enrollment Days
Parents interested in enrolling 6th or 7th grade students for the 2017-2018 school year, please come to one of the open enrollment days listed below:
Tuesdays 10:00 a.m. – 1:00 p.m. and 5:00 – 7:00 p.m.
Wednesdays 10:00 a.m. – 1:00 p.m. and 5:00 – 7:00 p.m.
Saturdays 10:00 a.m. – 12:30 p.m.
Please bring the documents listed below with you:
Two (2) proofs of residency. The school will accept an original
lease or mortgage statement, a recent (60 days or less) payroll stub,
a gas/water/electric bill, a 2016 W-2 form or an official letter from
the city housing authority, ACS or HRA with the residents’ name
and address stated. PLEASE NOTE THAT A CABLE BILL OR PHONE
BILL, A DRIVER’S LICENSE OR A PASSPORT IS NOT ACCEPTED AS
PROOF OF RESIDENCY.
Copy of your child’s last report card
An original certified copy of your child’s birth certificate (no copies)
A copy of immunization records (if possible)
Parent/Guardian’s Photo I.D. (Driver’ License, Non-Driver’s License,
Proof of guardianship (if applicable)
